| The Cardholder/ Driver's name, Company name, and the Vehicle Registration Number and Make will be embossed on the Card. The Card can only be used by the designated Driver for the designated vehicle. |
| Type B : The Non-Driver Card |

| Only the COMPANY name and the Vehicle Registration Number and Make will be embossed on the Card. It is most suitable if you have a pool of Drivers driving the same vehicle. |
| Type C : The Dual Card |

| Two Cards will be issued. A Payment Card and a Vehicle Card. Both Cards must be used together to complete a transaction. For the Payment Card, you have the option of either embossing the Company name only or the Company name and the Cardholder/ Driver's name on the card. |
2. State the Drivers' name where applicable 3. State the Vehicle Registration Number 4. State the Vehicle Make and Model 5. Indicate if truck tonnage is above 3.5 ton 6. State the department/ branch using the vehicle. (Applicable only if you would like to have a separate Cost Summary Report) 7. Indicate if odometer reading is required 8. Specify the product and services allowed 9. Specify the number of transactions per day allowed, the daily, weekly and monthly purchase limits. (If applicable).
